Self Service - ESR Forms

Employee access to personal data (ESR - SELF-SERVICE)

All employees will have access to their personal data held on the Electronic Staff Records System (ESR) via all PC’s in the Trust.


You are able to access personal information, name, date of birth and amend your home address and bank details.


You are able to access an “on-line pay slip and P60 (2013/14 tax year onwards).


“SmartCard” holders will access this information from the “295 Employee Self Service (Limited Access)” URP when they log in to ESR in the normal way from August 2014.


All staff that do not have a “SmartCard” will need to access your Trust email account to retrieve your log in and password to the ESR system. Please contact IT from the intranet (“man” icon at top of page) or phone 41130 if you are having issues accessing your Trust email account.


The log in and password are unique to each individual member of staff, please make sure they are kept safe and secure. You will need to login and password and need to contact  Tel No. 48935 or [email protected]  to re-set your password.

A Total Reward Statement (TRS) will be available via the ESR Self Service system where you will be able to access a yearly pension statement if you are a member of the NHS Pension Scheme. Further information regarding the content and access to the statement will be attached to your pay slip in September 2014.
